Q: "How much do personal assistance services cost?"
A: Please call our office for our hourly rates. Rates decrease if our services are provided for four consecutive hours or more per day.
Q: "When will you come to my house?"
A: This is a common question! We will provide your personal assistance services when you need us, and as agreed upon in your individualized service plan and consent form. Services may begin when your admission to Trinity Personal Care is complete and the appropriate documentation has been signed.
Q: "How long can my caregivers come to my home?"
A: As long as you need them to. We will revise your individualized service plan to custom fit your needs as they change.
Q: "When will I be discharged from Trinity Personal Care services?"
A: We hope you stay with us, but there are several different reasons or circumstances for a client's discharge from an agency. Most commonly, you will be discharged when you feel you no longer need assistance.
Q: "If I'm discharged from Trinity Personal Care, can I ever receive services again?"
A: Absolutely. Additionally, if your needs change and you also need physician ordered, intermittent skilled care, our sister agency, Total Patient Care Home Health, LLC, can help.
Q: "How do I make a referral to Trinity Personal Care?"
A: You may submit the referral form online or call our office and speak to one of our management team. We will schedule an appointment time for your complimentary admission to our agency. No physician's order is required for personal assistance services.
Q: "What are your office hours?"
A: Monday through Friday, 8am-5pm. The phone is answered 24 hours per day. After 5pm, the office phone is answered by an answering service, and the manager on call will receive your message and promptly return your call.
